PRACTICE CLOSURE

As a patient of Self Empowered Healing, you have given me the privilege and honor of contributing to your well-being. So it is with sadness that i let you know i will be retiring from Self Empowered Healing as of 31 July 2026. i look forward to creating a new personal lifestyle of well-being and the adventure of graceful aging. 

 

i will be available to see you for a final consultation of 30 or 60 minutes on a limited schedule through the end of July. If you are already scheduled for a visit after that, please contact me if you would like to be moved  up to my limited openings in July. If you wish to cancel a current appointment, please let me know as soon as possible so i can support those who still want care. If you have any medications you want refilled between now and 31 July 2026 please request them through the portal. If you have been seen in the last six months and labs are up to date, i can give you refills for another six months if appropriate so that you have time to find another physician. Your medical records will continue to be available to you through our Charm electronic health record portal https://phr2.charmtracker.com/login.sas until 31 December 2026. i strongly recommend you make digital or paper copies of any information in the portal, including laboratory reports, so that you can share these with your other healthcare providers.
